              <p>Radiotherapy plays a critical role in cancer treatment, requiring precise dose planning to maximize tumor control while minimizing damage to healthy tissues. This study explores advanced dose modeling techniques, specifically the Linear Quadratic (LQ) and Linear Quadratic Linear (LQL) models, to refine biologically effective dose (BED) calculations and enhance treatment personalization. Utilizing patient-specific data—such as fractionation schedules and organ-at-risk (OAR) constraints—these models incorporate radiobiological principles to predict outcomes, including normal tissue complication probabilities (NTCP).Through a series of clinical case studies, we assessed treatment planning variability using the Jaccard similarity index, revealing a striking lack of consensus among practitioners, with a mean agreement of only 10%. This finding underscores a significant gap between the theoretical potential of advanced modeling and its consistent clinical implementation. While these models offer promising advancements in personalized radiotherapy, variability in interpreting and applying their outputs presents a major challenge.To bridge this gap, we emphasize the urgent need for standardized protocols, comprehensive guidelines, and specialized training programs. Establishing these frameworks will reduce inter-observer variability, facilitate the seamless integration of dose modeling into clinical practice, and ultimately improve patient outcomes.</p>
